Housing Retention Is Foundational to Wellness
Housing Case Management
Sondercove Wellness focuses on supporting participants in securing, maintaining, and stabilizing safe housing. We recognize that mental health and substance use recovery are deeply connected to housing security, and our work addresses the practical and systemic barriers that often make stable housing difficult to obtain or sustain.
What Participants Can Expect
Participants can expect structured support navigating the housing system, identifying available options, and developing strategies to maintain stable housing. This includes assistance understanding leases, communicating with landlords, addressing housing-related barriers, and connecting with community housing resources.
Our approach acknowledges that housing instability disproportionately impacts Black communities and individuals navigating poverty, system involvement, and discrimination in rental markets. We work alongside participants to strengthen their ability to advocate for themselves while navigating these complex systems.
