Overview of ISDE Services at Sondercove Wellness
We view ISDE as a bridge between therapeutic insight and practical application. It’s where recovery takes shape in daily routines, personal choices, and long-term goals. Our ISDE services are peer-informed, trauma-responsive, and centered around the unique vision each person has for their life. We do not assume what skills someone "should" have—instead, we explore what they want to strengthen, learn, or reclaim.

How Services Are Typically Provided
ISDE services are offered in-home, in the community, virtually, or wherever the learning makes sense. Whether practicing how to grocery shop on a budget, preparing for a job interview, or walking through emotional regulation strategies in real time, we meet participants in the moments that matter. Our team includes staff with lived experience, cultural knowledge, and practical tools to support meaningful growth in a non-judgmental, real-life way.

What ISDE Services Can Include

Here are examples of support that fall under ISDE and are billable within the CCS framework:

Financial Literacy & Budgeting

  • Building and following a personal or household budget

  • Navigating online banking, setting savings goals, and managing debt

  • Understanding rent, utilities, taxes, and cost of living

  • Rebuilding credit or preparing for large purchases (e.g., vehicles, housing)

Daily Living Skills

  • Time management, routine-building, and personal organization

  • Meal planning, grocery shopping, and basic cooking

  • Navigating public transportation and community resources

  • Medication self-management and setting up reminders

Post-High School Planning

  • Exploring vocational and academic options after high school

  • Applying to colleges, apprenticeships, or training programs

  • Navigating applications for financial aid or scholarships

  • Preparing for campus life, independent living, or transitional housing

Employment Preparation (See “Employment Services for more information)

  • Resume and cover letter development

  • Practicing interviews and understanding workplace expectations

  • Navigating employment-related challenges like transportation or social anxiety

Relationship Building & Social Development

  • Strengthening communication, conflict resolution, and boundary-setting skills

  • Reconnecting with family, co-parenting support, or navigating chosen family dynamics

  • Addressing social anxiety and building confidence in group settings

  • Exploring healthy relationships and identifying red flags

Mental Health & Recovery Integration

  • Applying coping skills in daily life (e.g., DBT tools, emotion regulation, mindfulness)

  • Learning how to manage mental health symptoms in public, work, or school settings

  • Practicing strategies for relapse prevention or harm reduction in social environments

  • Enhancing awareness of internal triggers and building self-regulation routines

Technology Skills

  • Using smartphones, tablets, and computers for telehealth, job searches, and social connection

  • Navigating email, digital applications, and online safety

  • Creating wellness routines through apps and digital calendars

Parenting & Family Life Skills

  • Supporting participants who are parents with routines, communication, and scheduling

  • Navigating child-related appointments, school communication, and CPS involvement

  • Co-regulation and modeling healthy behaviors with children

  • Understanding lease agreements, maintaining a household, and resolving maintenance issues

  • Preparing for inspections, developing cleaning systems, and organizing shared spaces

  • Practicing communication with landlords and neighbors
